First Visit
Please bring the following to your first visit:
Driver’s license or other picture ID
Referral or prescription from your doctor. This is always required to make sure you receive maximum insurance coverage available for physical therapy services.
Insurance card
If you have a list of current medications available, we would appreciate your bringing a copy.
At this appointment we will also ask you to fill out a few brief forms to give us an idea of pertinent medical history and contact information. You can also fill these forms out before you come by going to the forms page and downloading the forms.
